Patient Eligibility
Our mobile wound care program is designed for patients who:

Have acute or chronic non-healing wounds -
Are home-limited due to mobility, chronic illness, or post-surgical recovery -
Require biologic or advanced therapies such as grafts or negative pressure wound therapy (NPWT) -
Need regular wound monitoring to prevent infection or hospitalization -
Receive home health services or reside in assisted living or skilled nursing environments
NOTE
Patients do not need to be fully homebound to qualify. If leaving the home is medically difficult or burdensome, our clinicians can deliver in-home wound care that meets Medicare criteria for coverage.

Your Health Allies provides services covered by Medicare Part B and most Medicare Advantage plans.
