If you or someone you love finds it difficult to travel to a doctor’s office due to mobility issues, transportation challenges, or medical complexity, you likely qualify for our mobile care program
How Qualification
Works
Becoming a patient with Your Health Allies is straightforward. After a brief intake conversation, our clinical team reviews your health history, medications, and living situation to confirm eligibility. Once approved, we schedule your first in-home visit and begin coordinating with your existing providers to ensure continuity of care.
